Birth of the child with unmarried parents at the time of birth of the child but acknowledge by the father/parents in the affidavit of admission of Paternity/Acknowledgment found at the back of the COLB or separate affidavit and has Affidavit to Use the Surname of the Father attached.

WHERE TO SECURE
Duly accomplished Form 102 (COLB) in four copies.
Hospital/Clinic where the child was born
NOTE: If the birth of the child was attended by a hilot, report to the clinic where the child was born.
Affidavit of Admission of Paternity/Acknowledgement found at the back of the COLB or in separate affidavit duly registered in the MCRO in four copies.
MCR Office and/or Notary Public
Affidavit to Use the Surname of the Father duly registered in the MCRO in four copies, if applicable
MCR Office and/or Notary Public
ADDITIONAL REQUIREMENTS IN CASE OF DELAYED REGISTRATION OF BIRTH
Any two of the following documents where we can base the Date and Place of Birth of the Child
– Baptismal Certificate – 1 Original copy or Certification
– School Records: Form 137E or OTR – 1 Original copy or Certified copy
– Income Tax Return of Parents – 1 Original copy or Certified copy
– Insurance Policy – 1 Original or Certified copy
– Voter Registration Record – 1 certified copy
– Barangay Captain’s Certification – 1 copy
Personal Copy or church where the child was baptized
Elementary, Secondary and Tertiary Schools where the child was enrolled.
Personal Copy or from Employer
Personal Copy or from the Insurance Company
Barangay Captain where the child was born.
PSA Negative Certification – 1 Original copy
Philippine Statistics Authority
Marriage Contract/Certificate, if married – 1 certified copy
PSA or MCRO
Affidavit of two-disinterested persons who might have witness/knowledge about the child’s birth – 1 original copy.
FEES TO BE PAID
1. Give the logbook to the client and assist them.
2. Verify and evaluates as to the authenticity and completeness of the documents submitted
2.1 In case of admission of Paternity/acknowledgement incorporated in the form.
2.1. Prepare and Administer oath on the affidavit of admission of Paternity/acknowledgement found at the back of the COLB.
2.1 In case where there is an affidavit to Use the Surname of the Father attached or Affidavit of Admission of Paternity/Acknowledgement attached in separate form.
2.2. Examine/Verify if the affidavit is duly signed and registered by the concern MCR.
2.3 In case of Delayed Registration
2.3. Prepare Notice of Posting.
NOTE: Posting Period is ten (10) calendar days to be posted in three (3) conspicuous places in the Municipal Bulletin Board
2.4 Record the document in the logbook, assign Register Number and forward to the MCR for registration.
2.5 Review and approved the documents for registration.
2.6 Return to ARO and encode in the CRIS and Philsys data base and record in the Registry Book.
3. Acknowledged receipt of the owner’s copy of the registered document by affixing his/her signature in the logbook.
3. Release the owner’s copy of the document.
