CHECKLIST OF REQUIREMENTS
WHERE TO SECURE
Duly accomplished Form 103 (COD) in four copies signed by the Medical Officer who have attended the death reviewed by the Municipal Health Officer and with certification of embalmer, if embalmed and affidavit for delayed registration found at the back of the Certificate of Death
Hospital/Clinic where the death occurred.
NOTE: If the death occurred in places other than hospital/clinic report immediately to the hospital/clinic where the death occurred.
In the absence of the Municipal Health Officer, the Mayor, Municipal Secretary or any member of the Sangguniang Bayan can sign the certification portion (Registration was made Pursuant to Section 91 of PD 856) and same is accepted for registration.
PSA Negative Certification – 1 Original copy
Philippine Statistics Authority
Barangay Captain’s Certification – 1 copy
Affidavit of two-disinterested persons who might have witness/knowledge about the death of the person – 1 original copy.
Burial Permit/cremation or other means of corpse disposal
Personal copy/Treasury Office/Crematory
FEES TO BE PAID
1. Give the logbook to the client and assist them.
2. Verify and evaluate as to the authenticity and completeness of the documents submitted
2.1. Prepare Notice of Posting.
NOTE: Posting Period is ten (10) calendar days to be posted in the Municipal Bulletin Board
2.2. Record the document in the logbook, assign Register Number and sign for registration
2.3. Record the document in the Register of Deaths and encode in the CRIS data base.
3. Acknowledge receipt of the owner’s copy of the registered document by affixing his/her signature in the logbook.
3. Release the owner’s copy of the document.
