For Births occurred outside the municipality. The registration was made Pursuant to Rule 20 of Administrative Order No. 1, series of 1993.

CHECKLIST OF REQUIREMENTS

WHERE TO SECURE

Duly accomplished Form 103 (COD) in four copies signed by the Medical Officer who have attended the deceased reviewed by the Municipal Health Officer and with certification of embalmer, if embalmed and affidavit for delayed registration found at the back of the Certificate of Death

Hospital/Clinic where the death occurred.

NOTE:  If the death occurred in places other than hospital/clinic report immediately to the hospital/clinic where the death occurred.

PSA Negative Certification – 1 Original copy

Philippine Statistics Authority

Barangay Captain’s Certification – 1 copy

Affidavit of two-disinterested persons who might have witness/knowledge about the death of the person – 1 original copy.

Notary Public

Burial Permit/cremation or other means of corpse disposal

Personal copy/Treasury Office/Crematory

Affidavit of the applicant declaring among other things the facts of death and the reason why said death was not recorded/registered in the city/municipality where it occurred. The affidavit shall be attested by two (2) witnesses and shall serve as an application for registration.
