It is the provision of limited emergency financial assistance to individuals and families in utmost crisis situations, victims of calamities and have inadequate resources.

1.    Hospitalization

1.1   Certification of Indigency (1 original, 2 photocopy)

1.2   Medical Abstract/Certificate(1 original,2 photocopy)

1.3   Hospital bill(1 original, 2 photocopy)

2.    Laboratory exams and other medical procedures

2.1   Certification of Indigency (1 original,2 photocopy)

2.2   Doctor’s recommendation for laboratory exam or other medical procedures( 3 photocopy)

3.    Burial assistance

3.1   Death certificate ( 3 photocopy)

3.2   Certification of Indigency (1 original,2 photocopy)

3.3   One valid ID card of client (3 photocopy, bring original for verification)
